Policy Summary
Medicare covers supervised exercise therapy (SET) for beneficiaries with peripheral artery disease who have intermittent claudication when delivered as a 30–60 minute therapeutic exercise program in a hospital outpatient setting or physician's office under qualified supervision. National coverage permits up to 36 sessions over 12 weeks when program, personnel, supervision, and referral/documentation requirements are met; MACs may authorize an additional 36 sessions with a second referral.
