Treatment of Obesity

NCD 38 no longer contains coverage criteria for treatment of obesity because section 40.5 was removed from the NCD Manual and incorporated into NCD 100.1. Consult NCD 100.1 for the current indications, limitations, documentation requirements, and frequency limits for obesity treatment coverage.
