Policy Summary
Medicare covers biofeedback therapy only when reasonable and necessary for muscle re-education or to treat pathological muscle abnormalities (spasticity, incapacitating muscle spasm, or weakness) after conventional treatments (heat, cold, massage, exercise, support) have failed. Biofeedback is excluded for ordinary muscle tension states and for psychosomatic conditions and must meet general Medicare physical therapy coverage requirements (Medicare Benefit Policy Manual, Chapter 15).
