Policy Summary
Biofeedback-assisted pelvic muscle exercise is covered when used by a practitioner in an office or facility to treat stress or urge urinary incontinence in cognitively intact patients who have failed a documented 4-week trial of pelvic muscle exercises. Home use is excluded, and contractors retain discretion to cover biofeedback as an initial treatment modality. Documentation must show completion of the 4-week PME plan and no clinically significant improvement before coverage.
