Ultrafiltration, Hemoperfusion and Hemofiltration

Hemoperfusion is covered for life‑threatening drug overdose and for aluminum toxicity when used with deferoxamine; it is not covered to improve chronic hemodialysis or when used with deferoxamine for iron overload. Hemofiltration is a covered alternative for ESRD patients (requiring arteriovenous access) and may be performed three times weekly in facility or at home; ultrafiltration is covered to remove excess fluid not manageable during regular hemodialysis, but predialysis ultrafiltration does not warrant an additional facility payment unless documented as unable to be performed during dialysis.
