Policy Summary
Medicare may cover laser procedures for medical indications when the specific laser device is FDA-approved and there is no explicit noncoverage instruction; coverage is subject to MAC determination of reasonable and necessary. Laser use to alter, revise, or destroy tissue is considered surgical and coverage is limited to practitioners trained in the surgical management of the treated disease or condition.
