Refractive keratoplasty and radial keratotomy performed solely to correct common refractive errors (e.g., myopia, hyperopia) are not covered as substitutes for eyeglasses/contact lenses and as cosmetic surgery. Keratoplasty that treats specific corneal abnormalities (for example, phototherapeutic keratectomy to remove scar tissue) may be covered as medically necessary under §1862(a)(1)(A), and laser/keratoplasty procedures are covered only when performed by practitioners with approved ophthalmologic surgery training; certain exceptions in connection with cataract surgery require case-by-case review.
