Partial Thromboplastin Time (PTT)

PTT is covered primarily to monitor and guide therapeutic unfractionated heparin dosing, to evaluate patients with signs or symptoms of bleeding or thrombosis, and to assess individuals with personal histories or conditions affecting the intrinsic coagulation pathway. Routine use for warfarin monitoring is not covered except during heparin-warfarin transitions or when PT is markedly prolonged; pre-procedural screening without signs/history is generally not medically necessary. Documentation must separately justify PTT (and PT if ordered) and show clinical necessity such as symptoms, history, or changes in anticoagulant management.
