Policy Summary
Invasive intracranial pressure monitoring (NCD84) is covered when deemed reasonable and necessary for an individual patient, particularly for neurosurgical or neurologic patients in specialized ICUs. Typical clinical indications include head (traumatic) injury, subarachnoid hemorrhage, intracerebral hemorrhage, Reye’s syndrome, and posthypoxic, metabolic, or viral encephalopathies; coverage is not provided when the procedure is not reasonable and necessary.
