Policy Summary
Conventional therapies for psoriasis (topical agents, actinotherapy, and Goeckerman treatment) are covered. PUVA (psoralen + UVA) is covered only for intractable, disabling psoriasis after documented failure of conventional treatments; reimbursement for PUVA is ordinarily limited to 30 days and to amounts comparable to other photochemotherapy, with required documentation of nonresponse and documented improvement for extended treatment.
