Blood glucose testing is covered for management and evaluation of diabetes mellitus, impaired fasting glucose (FPG 110-125 mg/dL), insulin resistance, hypoglycemia disorders, patients on medications affecting carbohydrate metabolism (e.g., glucocorticoids), and a variety of clinical conditions or symptoms that may be caused by glucose abnormalities. For stable, non-hospitalized patients unable/unwilling to self-monitor, quantitative testing is generally limited to up to four times per year unless more frequent testing is justified and documented; screening tests and specific frequencies must follow 42 CFR 410.18 and Claims Processing Manual guidance.
