Policy Summary
Medicare covers intraoperative ventricular mapping only when performed intraoperatively to (1) localize accessory pathways in WPW and other preexcitation syndromes, (2) map atrial/ventricular activation for drug-resistant supraventricular tachycardias, (3) delineate His bundle or bundle branch anatomy during corrective surgery for congenital heart disease, or (4) guide surgical treatment of refractory ventricular tachyarrhythmias. Uses outside these specified indications are not covered. The policy text does not specify documentation requirements or frequency limits.
