This policy addresses optical coherence tomography (OCT) of the anterior eye segment for imaging and measurement of the anterior chamber angle, cornea, ciliary body and related structures to evaluate conditions such as suspected narrow or occludable iridocorneal angles/angle‑closure glaucoma, pre‑ and post‑refractive surgery assessment, corneal disease, intraoperative surgical guidance, and device/complication evaluation. However, anterior‑segment OCT is considered experimental/investigational and generally not covered because of device- and wavelength‑specific imaging limitations (e.g., 1.3 µm iris blockage, posterior‑segment OCT not optimal for anterior structures), variable resolution, lack of FDA clearance for some devices, and insufficient evidence of clinical validity and usefulness compared with established tests (e.g., gonioscopy, ultrasound biomicroscopy).
