Endothelial keratoplasty (selective replacement of diseased corneal endothelium, e.g., DSEK/DSAEK or DMEK/DMAEK) is covered to treat endothelial dysfunction. It is indicated for conditions such as Fuchs endothelial dystrophy, aphakic/pseudophakic bullous keratopathy, corneal edema from endothelial failure, iridocorneal endothelial syndrome, and failed/rejected prior endothelial grafts; coverage is subject to the member’s benefit plan, requires adequately trained surgeons, and femtosecond- or femtosecond-plus-excimer laser–assisted endothelial keratoplasty is considered experimental and not covered.
