This policy covers biofeedback therapy (e.g., EMG or thermal biofeedback) as a medically necessary part of overall treatment and preventive care for migraine and tension‑type headaches. Coverage is generally limited to 20 office‑based sessions (additional sessions require medical‑necessity review), home use is not covered, non‑migraine/tension‑type headaches (e.g., cluster) are considered experimental, and plan benefits and limited/mixed evidence (especially for chronic tension‑type and pediatric populations) may affect coverage.
