This policy covers magnetoencephalography (MEG) and magnetic source imaging (MSI) for noninvasive functional brain localization — for example, lateralizing language as an alternative to the Wada test and preoperative localization of epileptic foci and eloquent cortex in patients undergoing brain resection (e.g., intractable epilepsy, brain tumors, AVMs). Coverage is limited to specified indications; MEG/MSI requires SQUID-based gradiometer coils in a magnetically shielded room, is considered investigational/not covered for indications not listed, and the policy is inactive for current claims adjudication.
