Covers use of anesthetic agents (e.g., intramuscular/intravenous ketamine, intravenous lidocaine, and local/regional lidocaine) for psychiatric disorders and selected pain indications such as treatment‑resistant depression, OCD, PTSD, chronic neuropathic pain, migraine, fibromyalgia, CRPS and related conditions. Coverage is restricted to uses, doses and durations supported by FDA labeling or authoritative references; many applications (including IM/IV ketamine and IV lidocaine for numerous psychiatric and chronic pain indications) are considered experimental/investigational and not covered, require careful monitoring due to significant adverse‑event risks, and are subject to member benefit and state regulatory limits.
