Liver transplantation and combined liver‑kidney transplantation when medically necessary for end‑stage liver disease and related indications (e.g., alcoholic liver disease, viral and autoimmune hepatitis, metabolic/genetic disorders, select primary hepatocellular carcinoma per policy criteria, and other listed causes) provided the individual meets Organ Procurement and Transplantation Network (OPTN) selection criteria. Coverage requires meeting OPTN criteria and program requirements (commonly documented abstinence from alcohol/drugs, evaluation for psychosocial suitability, and absence of untreated systemic infection or uncontrolled/active malignancy). Major exclusions/limits include procedures or organs linked to forced organ harvesting (certain countries), transplants for extrahepatic HCC or other contraindications, and several indications (eg, metastatic neuroendocrine tumors to liver, intrahepatic cholangiocarcinoma) deemed experimental or unproven.
