Coverage requires adequate cardiopulmonary status, documented compliance with medical management, and plan-specific authorization; exclusions include active or high‑risk malignancy, untreated systemic infection, psychosocial/substance‑use barriers to adherence, and any transplant or organ procured or performed in countries implicated in forced organ harvesting, with final determinations governed by the member’s benefit plan.
