Radiofrequency ablation (RFA) of primary or metastatic liver tumors for treatment of inoperable hepatocellular carcinoma (single lesion ≤5 cm or up to 3 nodules <3 cm), as a bridge to transplant, and for hepatic metastases primarily from colorectal cancer or select neuroendocrine tumors when surgical resection is not feasible. Coverage requires lesions be inoperable and meet size/number criteria, excludes cases with extrahepatic disease or operable HCC, and considers use outside these limits (e.g., other primaries, >3 nodules, larger tumors) experimental/investigational; member benefit plans and state rules govern final coverage.
