This policy addresses use of a drug‑coated urethral balloon (paclitaxel‑coated) to treat obstructive urinary symptoms from anterior/bulbar urethral strictures and for urethral dilation in benign prostatic hyperplasia (BPH) in adult men. Coverage is limited to men (generally ≥50 for BPH) with short anterior strictures (study devices/indications ≤2–3 cm, often after 1–4 prior endoscopic procedures), and major limitations include limited nonrandomized/short‑term evidence, classification as experimental/unproven for many indications, exclusion of women and pediatric/posterior strictures, and applicability subject to each member’s benefit plan.
