This policy covers interspinous and interlaminar stabilization/distraction devices (spacers) implanted between spinous processes or lamina to distract neural foramina, limit extension, and provide dynamic stabilization or adjunctive support for symptomatic lumbar spinal stenosis with neurogenic claudication (including selected patients with low‑grade spondylolisthesis). Coverage is limited to select indications after failure of conservative therapy and subject to member plan benefits; use of these devices as stand‑alone procedures or routine interlaminar stabilization after decompression is considered experimental, investigational, and/or unproven and not covered.
