This policy addresses isolated facet joint fusion to stabilize the spine and reduce facet‑mediated pain (including when performed with posterior lumbar fusion), and lists techniques such as TruFUSE and NuFix allograft approaches as well as screw-and-bone‑graft fusion for low‑grade lumbar/lumbosacral spondylolisthesis and facet pain refractory to conservative treatment. Major limitations are that isolated facet fusion using prepared allograft dowels (including TruFUSE/NuFix) is considered experimental/investigational and not covered due to limited, short‑term and low‑quality evidence with poor radiographic fusion rates; the policy is inactive, not for claims adjudication, and coverage remains subject to member benefits and local carrier or Medicare discretion.
