This policy addresses decompression of the intervertebral disc using laser energy (laser discectomy) or radiofrequency coblation (nucleoplasty) for patients with discogenic low back pain, radiculopathy, and contained cervical, thoracic or lumbar disc herniations. Major limitations require failure of ≥12 weeks of conservative therapy (and positive nerve‑conduction studies for cervical procedures), and the policy generally considers laser discectomy and radiofrequency coblation experimental/investigational and not covered unless high‑quality evidence demonstrates a net health benefit with appropriate patient selection and follow‑up.
