Axial Lumbosacral Interbody Fusion

Axial lumbosacral interbody fusion (e.g., AxiaLIF) for anterior access to the L4–S1 disc spaces is addressed for indications including degenerative disc disease, spondylolisthesis, spinal stenosis with instability, lumbar scoliosis, adjacent‑level disease, recurrent symptoms after discectomy, and select revision cases. Major limitations/requirements are that use is restricted to the L4–S1/L5–S1 levels, must be supplemental anterior fixation used with a legally marketed facet or pedicle screw system, cannot address intracanal pathology, carries unique risks (e.g., bowel/vascular injury and increased fluoroscopy), and the policy deems the technique experimental/investigational and not covered.
