Viscocanalostomy and Canaloplasty

This policy covers canaloplasty and related transluminal viscodilation procedures to lower intraocular pressure in adults with chronic primary open‑angle glaucoma who have failed medical therapy and are not candidates for other filtration or drainage surgeries. Viscocanalostomy is considered investigational, and canaloplasty is limited by modest IOP lowering (often only to the mid‑teens), limited long‑term durability (~50% standalone success at 2 years), is for adult open‑angle disease only, and may not be suitable for patients requiring very low target pressures or with angle‑closure glaucoma.
