This policy covers heat and cold therapy devices, including FDA‑approved scalp cooling systems for prevention of chemotherapy‑induced hair loss and localized cold/heat devices used post‑operatively, after acute injury, before/after physical therapy, and for pain relief, muscle spasm, circulation improvement, and athletic applications. Coverage is determined by the member’s benefit plan (scalp cooling limited to FDA‑approved use during chemotherapy, with an Illinois statutory pediatric coverage exception for certain fully insured plans effective 2025‑01‑01); many items (motorized water‑circulating pads, vaso‑pneumatic systems, heat/cold wraps, non‑electric pads and routine ice/gel packs) are considered convenience/OTC items or not DME and are not medically necessary, and use is contraindicated with active bleeding, temperature insensitivity, or inability to communicate pain.
