This policy addresses nonpharmacologic treatments for rosacea—including laser and light-based therapies (vascular lasers, IPL), dermabrasion, chemical peels, electrosurgery, and procedures for rhinophyma (CO2 laser, surgical debulking)—to reduce persistent erythema, telangiectasias, papules/pustules and improve cosmetic appearance. It applies to patients who are intolerant of, decline, or are unresponsive to pharmacologic therapy, but these modalities are considered experimental/investigational and not covered due to limited low-quality evidence; surgical treatment of rhinophyma is reserved for symptomatic cases (e.g., airway obstruction) or when malignancy is suspected.
