This policy addresses stationary ultrasonic diathermy devices (therapeutic ultrasound/low‑intensity continuous ultrasound or sustained acoustic medicine) for noninvasive generation of deep therapeutic heat to promote soft‑tissue healing and pain relief in musculoskeletal conditions such as upper shoulder/neck/back injuries, knee joint disorders including mild–moderate osteoarthritis, muscle spasms, joint contractures, and other soft‑tissue injuries. However, the devices are generally considered experimental, investigational, or unproven and not covered except where state law overrides (notably an Illinois-only coverage carve‑out for certain fully insured pediatric plans effective 2025‑01‑01); use is contraindicated or high‑risk for patients with implanted medical or metal devices, active malignancy or healing fractures, pregnancy, and other specified conditions, and available evidence is limited and heterogeneous.
