UnitedHealthcare covers reproductive carrier screening panels of up to 6 genes, and up to 15 genes only if the individual or partner has Ashkenazi Jewish ancestry (≥1 parent/grandparent) or a biological first‑ or second‑degree relative affected by a condition on the panel; panels of 16+ genes and carrier screening for other indications (including adult‑onset conditions) are unproven and not medically necessary. Pre‑test genetic counseling is strongly recommended and documentation of counseling and the qualifying ancestry/affected‑relative must be available (medical records may be requested) to support coverage.
