Implantable loop recorders (ILRs) are medically necessary for evaluating suspected cardiac arrhythmias (e.g., post-cryptogenic stroke AF, ventricular arrhythmia, high-risk structural/infiltrative disease, recurrent/unexplained infrequent syncope, or abnormal EP/tilt testing) and for replacement if beyond useful life/irreparable, provided noninvasive monitoring was contraindicated or yielded nondiagnostic results after ≥2 weeks and appropriate medical documentation is supplied. Consumer-grade wearable cardiac self-monitoring devices and related downloading hardware/software are excluded as not medically necessary and considered convenience items.
