UnitedHealthcare covers specified infertility diagnostic tests, treatments, ART services, and short‑term (≤1 year) cryopreservation of sperm, mature oocytes, and embryos when the member’s specific plan includes infertility or iatrogenic fertility‑preservation benefits, but excludes elective preservation, embryo transfer for preservation cycles, long‑term storage (>1 year), donor procurement/storage costs, surrogate fees for non‑covered carriers, self‑injectable infertility drugs, and services beyond plan maximums. Coverage requires the member plan to include infertility benefits, medical-record documentation meeting clinical criteria (including evidence of planned gonadotoxic therapy for iatrogenic preservation), any required prior authorization, submission of diagnosis code Z31.84 for iatrogenic preservation, and is limited to one preservation cycle per covered person.
