This bulletin updates UnitedHealthcare Commercial Medical Policies and Medical Benefit Drug Policies (including 2026 CPT/HCPCS code changes) and addresses services such as sleep studies for sleep disorders, surgery of the hip, total artificial disc replacement for degenerative spinal disease, and vagus and external trigeminal nerve stimulation for certain neurologic conditions. Coverage is determined by the member’s benefit plan and applicable law, detailed code and clinical-coverage edits will be provided in the January 2026 bulletin, inclusion here does not guarantee coverage (investigational/not medically necessary services may be excluded), and providers must obtain written member consent before billing for non‑covered services while UnitedHealthcare may review clinical evidence before making coverage determinations.
