UnitedHealthcare covers percutaneous neuroablation (chemical or radiofrequency) as medically necessary for pancreatic cancer pain, severe cancer pain, and trigeminal neuralgia, while cold neuroablation (cryoneurolysis/cryoablation) is considered experimental and not addressed. Coverage requires meeting InterQual® percutaneous neuroablation criteria, alignment with the member’s specific benefit plan and supporting medical documentation (diagnosis, prior treatments for TN, procedure type), and listed CPT/diagnosis codes do not guarantee payment.
"Medical records documentation may be required to assess whether the member meets the clinical criteria for coverage but does not guarantee coverage of the service requested."
