UnitedHealthcare covers ankle surgeries (arthrodesis, arthroscopy, arthrotomy, and total ankle replacement) as medically necessary only when the member meets the cited InterQual® clinical criteria, while osteochondral allograft/autograft (OCA/OAT) for ankle cartilage defects is considered unproven and not medically necessary.
