Policy Summary
UnitedHealthcare considers surgical procedures for prevention or treatment of lymphedema (e.g., ARM, liposuction/lipectomy, LVA/LVB, VLNT and other microsurgical techniques) unproven, not medically necessary, and therefore not covered. Conservative management with complete decongestive therapy is required first-line, and coverage is determined by the member-specific benefit plan and applicable laws (the policy lists no circumstances that authorize these surgeries; Medicare Advantage plans may adopt separate evidence‑based rules).
