Policy Summary
Medicare Advantage covers hysterectomy for abnormal uterine bleeding and uterine fibroids, and uterine artery embolization (UAE) for fibroids and severe dysfunctional bleeding when deemed medically necessary, provided there is appropriate clinical documentation. MRI-guided focused ultrasound ablation is also covered for leiomyomata within specified volume limits. However, coverage is limited to cases related to these conditions, and UAE is not covered when not medically appropriate for the patient.
