Walkers are covered only when the member meets the InterQual® Medicare: Post Acute & Durable Medical Equipment — Walkers clinical criteria and the member-specific UnitedHealthcare benefit plan allows it; listing HCPCS codes does not guarantee coverage and walkers are not covered if InterQual or plan criteria are not met. Coverage requires documented medical necessity per InterQual and the member’s benefit plan (e.g., clinical notes and functional/ambulation assessment), adherence to applicable federal/state mandates and other DME policies, and MA plans may apply their own determinations when no NCD/LCD exists.
