Serum homocysteine testing is covered only for suspected vitamin B12 or folate deficiency and for risk stratification for conditions supported by the ICD-10-CM codes listed in LCD L34419. Coverage is limited to a single lifetime test for initial risk stratification, with subsequent testing permitted only if appropriate treatment does not correct the anemia or symptoms; documentation of diagnosis, prior treatment, and persistent findings is required.
