Nerve conduction studies (NCS) and electromyography (EMG) are covered consistent with LCD L34859; NCS without NEMG is allowed for carpal/tarsal tunnel (specified ICD-10 codes), for patients with significant lymphedema (I89.0), and for patients on anticoagulants (Z79.01/Z79.02). Non-covered services include portable non‑waveform EDX devices, psychophysical perception tests, wearable movement recordings, and SEMG/macro‑EMG; non-covered tests must be billed with CPT 95999 and include a claim description. Documentation must support ICD-10 and CPT/HCPCS coding, medical necessity, provider signature, and include waveforms/reports when requested; frequency limits apply per CPT Appendix J daily maxima and repeat-study limits of two or three studies per 12 months depending on diagnosis.
