Billing and Coding: Visual Field Examination

Coverage for visual field examinations is permitted when the service is reasonable and necessary as defined by LCD L33766. Providers must maintain complete, legible medical records that support selected ICD-10-CM and CPT/HCPCS codes, include a responsible clinician's signature, and include an ordering physician's statement of clinical indication/medical necessity; non-covered services must not be billed as covered and require the appropriate modifier when billed.
