This billing/coding guidance requires use of CPT code 81410, 81411, or 81479 (only one may be billed) with exactly 1 unit of service. The appropriate DEX Z-Code must be entered adjacent to the CPT code on specified Part A and Part B claim fields (Loop 2400/SV101-7/Box 19 for Part B; SV202-7/Block 80 for Part A) and an appropriate ICD-10-CM diagnosis code must be reported; see MolDX LCD L39989 for clinical indication criteria.
