Genetic testing for heritable thoracic aortic disease is covered for germline testing when the patient has an aortic root or ascending aortic dilation/aneurysm/dissection and at least one qualifying feature (age <60, syndromic features of Marfan/Loeys-Dietz/vascular Ehlers-Danlos, or relevant first- or second-degree family history). Coverage requires documented offer of genetic counseling, that the test includes minimum guideline-based gene content per ClinGen and ACC/AHA, that single-variant testing is limited to known familial variants, and that the assay has completed a MolDX Technical Assessment; tests with unvalidated or unclear-content and those conflicting with repeat-testing policy L38351 are not covered.
